CREMATORY

Final Disposition

Miller Mortuary offers cremation services to funeral homes and other mortuaries. 

​

Documents required are the following:

​

  • MMS Authorization for Cremation

  • Burial Transit Permit

  • Justice of the Peace Release stating that an autopsy was or was not needed from the county of death

  • If a JP Release is not available a certified death certificate *a certified death certificate takes roughly 3 weeks

AUTHORIZATION FOR CREMATION & DISPOSITION

Miller Mortuary & Crematory Services

  • Miller Mortuary requires that our Authorization for Cremation & Disposition must be signed by the legal next of kin to proceed with cremation.

BURIAL TRANSIT PERMIT

Texas Department of Health Bureau of Vital Statistics

  • Miller Mortuary requires a Burial Transit Permit be completed by the funeral home with Miller Mortuary as the crematoriam designated. The form must be signed by the local regisrtrar and the funeral director.

AUTHORITY TO CREMATE

Justice of the Peace

  • This form must be accompanied by Authority to Cremate signed by proper relative or legal representative of the deceased together with cremation permit from Bureau of Vital Statistics.

COUNTY AUTHORITY TO CREMATE

Miller Mortuary County Authority

  • Miller Crematory requires that proper county authority sign an authority to cremate before cremation can take place. Complete if an autopsy has been refused, but the county has granted permission to proceed with the cremation. If the proper county authority does not sign, a death certificate will need to be certified to proceed.

APPOINTMENT OF AGENT

Control Disposition of Remains

  • Agent can controll disposition in accordance with Section 711.002 of the Health and Safety Code.
